You can order your contact lens replacements through laEye Optical Online Express. Please fill out this form and press the Submit button below. Your contact lens order will be confirmed by email, then charged to your credit card and mailed to your address on file using your current prescription. Remember, laEye Optical cannot refill an expired prescription.  You may also visit our online store.

If your prescription is older than one year, you should schedule an appointment with Dr. D'Antonio's office. The * indicates where information is required to properly handle your request.
